Two details behind those numbers matter when you compare quotes. Novo Nordisk defines a month of Wegovy® as one box of four pens, and a month of Ozempic® as one box holding a single pen. A month of tablets is one bottle of 30.

Weight loss injection costs are still changing. Novo Nordisk has announced a new list price of $675 a month across Wegovy®, Ozempic®, and Rybelsus®, effective January 1, 2027. That change affects people whose out-of-pocket cost is tied to list price, such as those on high-deductible plans, and does not change the self-pay figures above.

Semaglutide Injection Cost Breakdown by Type

Semaglutide reaches patients under three brand names and through compounding pharmacies. Each route carries its own price and its own coverage rules.

Brand-Name Ozempic® Cost

Ozempic® is FDA-approved to improve blood sugar control in adults with type 2 diabetes. clinicians sometimes prescribe it off-label for weight loss, and that distinction has a direct effect on what insurance will pay.

Through NovoCare Pharmacy, new self-pay patients pay $199 a month for their first two fills at the 0.25 mg and 0.5 mg doses. That introductory offer runs through December 31, 2026, and applies only to patients new to the savings offer or to NovoCare Pharmacy.

After those first two fills, the price moves to $349 a month for the 0.25 mg, 0.5 mg, and 1 mg doses. The 2 mg maintenance dose costs $499 a month, which makes Ozempic® the one semaglutide product where a dose increase raises your bill.

Brand-Name Wegovy® Cost

Wegovy® cost starts from the semaglutide brand approved for chronic weight management in adults and in children aged 12 and older, per its FDA label. Its list price is $1,349.02 per package, which is what a retail counter charges when no savings offer is applied. 

Almost nobody pays that figure now. New self-pay patients pay $199 a month for the first two fills at the starter doses, then $349 a month for every standard pen dose from 0.25 mg through 2.4 mg.

The higher-dose Wegovy® HD 7.2 mg pen costs $399 a month. Unlike Ozempic®, the standard Wegovy® doses all carry the same price, so moving from 0.5 mg to 2.4 mg does not change what you pay.

Novo Nordisk also extended the $349 price to more than 70,000 pharmacies through a savings offer you enroll in before you fill.

Personalized GLP-1 Injection Cost

1. Where you buy it: Manufacturer-direct pricing runs well below the retail list price for an identical product. The same Wegovy® box can cost $349 or $1,349 depending on whether a savings offer was applied.
2. Which brand you are prescribed: Wegovy®, Ozempic®, and Rybelsus® all contain semaglutide, but each carries its own list price and its own self-pay tier.
3. Why it was prescribed: Most plans cover semaglutide for type 2 diabetes. Far fewer cover it when the reason on the prescription is weight loss.
